    <description>I am trying to locate any video/DVD (or even audio) on the topic of
Situation-Background-Assessment-Recommendation, sometimes called
situational briefing model. I have spoken to the folks at the Institute
for Healthcare Improvement with no luck. A few of the titles in
Locatorplus look promising, but I have to follow-up with the video
producers as the descriptions are not specific enough to tell me what I
need to know about the video contents.

You need to search the Medlib Archives.  I posted 
a long summary of answers to a similar question 
on January 18, 2006.  My subject line read:

Be sure to carefully read the license agreement and be very clear with
the publisher about what your users expect. We have had problems with
this publisher and unwritten rules about usage. For example, they have
shut off access to all of our users because someone downloaded more than
10 documents at one time. It was legitimate clinical use but the
publisher made it clear that this would happen anytime more than 10
documents were downloaded. The info in the database is good, it's the
publisher who is a problem.

I don't have the e version, only the print, but 
it's considered the 'gold standard' in herbal med.

We've had Natural Medicines for quite some time 
and our pharmacists as well as physicians and 
nurses use it and like it very much.
Its been a while since I checked statistics but 
last time I did--it had a very high usage 
rate.  Much higher than I had expected.
Pharmacists at our primary care clinic love it.

We don't have any other herbal/supplement 
resources--our other electronic pharmacy database is Micromedex.

It's an excellent tool, but a library site license is not cheap; I
believe the last quote I had for our school was in the $1200 range.
There is also a print version which is a great deal--I think about $95.

We had a brief subscription to this database and
it was very comprehensive and informative. It
cited evidence-based studies, drug interactions
and much more information including pictures of
the plants if necessary, common names, etc.
It is very expensive though and we could not justify
the cost with such limited usage as our doctors gave
it. Our cost was $1400/year
It would have been a great consumer education tool.

We have had a subscription to this database for 3 years. It really is a
good resource because so many patients take all kinds of
non-prescription "remedies". It supplies information on any drug
interactions and more.

I purchased this one year (2003), but I don't believe it got much use. I
still have the book format that we received with our online subscription. I
like it, but like I mentioned, it did not get much use.
This doesn't help much!

I work in a public library and we originally had 
the Natural Medicines Professional database. As a 
librarian, I loved it. It was extremely 
comprehensive and the user is able to link to the 
references in PubMed. They have an extensive list 
of brand name products, which has been helpful. 
There is also an interaction cheker which allows 
you to check interactions between natural 
products and prescription &amp; OTC medications. 
However, the information provided was too complex for most of our patrons.

I switched to the Consumer version which does not 
provide as in-depth information, but is much 
easier for the general public to use (in terms of 
readability). The Consumer version still offers 
the same features as the Professional. When I 
teach classes on using the health databases 
(which are attended mostly by seniors) everyone 
is very excited about the information here. 
However, there are a lot of options on the search 
page which I feel poses some difficulty for 
people who are not as comfortable using a computer.

This database is very expensive, but I feel it is 
money well spent: the information is 
evidence-based, easy to read and I have been able 
to find obscure natural products for patrons.

Our Pharmacists say this is the best source for 
herbals - especially good for herb-herb, 
drug-herb, lab-test-herb interactions and the 
brand names the herb could be sold under...

One of our physicians, an Alternative specialist, prefers this one over
Natural Standards.  If you click into the Medlib-l archives, you will
find  a summary and some comparisons.  There is an article in MRSQ on
Natural Standards - have not as yet checked for the Natural Database (I
think it is produced by Harvard).

We have used it for two or three years and we like it very much.  I like the
clear organization of the monographs and the copious references to studies
in the literature.  It seems easier to use than the Review of Natural
Products from Facts and Comparisons, though I really do like eFacts for most
other things.

And finally a comment from one of my own 
Physicians:  I have used the database for some 
time and find it a good balance of evidence based 
and clinician friendly.  I am familiar with the 
resources you have mentioned (for HDIs as well as 
the IBIDS) through my directorship of the Scripps 
Clinic Natural Supplements An Evidence Based 
Update yearly conference as well as being a 
member of the NIH ODS review committee for 
significant advances in Dietary Supplement 
research.  I find the IBIDS helpful for 
literature review, but not as helpful in 
providing the immediate answers regarding safety, 
efficacy for a said supplement or combination of 
med/herb.  I have also utilized a number of HDI 
searchable databases (micromedix, up to date, 
facts and comparisons, etc) and find the natural 
database the most comprehensive and frequently updated.

    <description>Dear Medlibbers,
My patron is sure there must be something on postoperative chemoradiation
(chemoradiotherapy, radiochemotherapy) for esophageal cancer.  I have found
every other combination, including a meta-analysis (BMC Medicine 2004)
which lists every other possible combination of therapies.  There is
postoperative chemoradiation for gastric cancer, but he wants something
specific to esophageal cancer.  I have mainly searched PubMed, also Google,
ASCO abstracts and clinical trials.gov.
Any thoughts from those who do a lot of cancer searches?  Maybe it is not
really there...

Others mentioned once were:

HP Scanjet 5590

Many people suggested adjusting the settings on Adobe and/or breaking up
the article into sections.
I'm going to submit a request for a new scanner and cross my fingers and
hope I get it.
